Vägen till ett framgångsrikt teamarbete vid en oväntat svår luftväg

Abstract

Bakgrund: Vid genomförande av generell anestesi är en fruktad komplikation hypoxi till följd av ofri luftväg vilket kan få förödande konsekvenser för patienten. För att öka patientsäkerheten är det av vikt att teamarbetet fungerar optimalt och här är kommunikationen en viktig del. Det finns föga forskning kring anestesisjuksköterskans upplevelser av oväntat svår luftväg och kommunikationens betydelse, vilket medför att studier i ämnet är av intresse. Vid genomförande av anestesi krävs handlingsberedskap för svåra luftvägar, vilket är en del i anestesisjuksköterskans kompetensområde. Syfte: Syftet med studien var att belysa anestesisjuksköterskans erfarenhet av teamarbetets betydelse för att hantera en oväntat svår luftväg. Metod: För att erhålla en djupare förståelse kring upplevelser i ämnet valdes en kvalitativ metod. Data samlades in genom två fokusgruppsintervjuer med totalt åtta deltagande anestesisjuksköterskor. Materialet som erhölls transkriberades och bearbetades med hjälp av kvalitativ innehållsanalys. Resultat: Totalt tre kategorier med tillhörande sex underkategorier utformades, dessa beskriver anestesisjuksköterskans upplevelse av att hantera en oväntat svår luftväg. Huvudkategorierna som utkristalliserade sig var; kommunikation, teamets kompetens samt förberedelser. Slutsats: Anestesisjuksköterskorna värdesätter kommunikation och teamsamarbete högt. Kommunikationen såg olika ut beroende på hur väl arbetslaget kände varandra och att effektiv kommunikation var en förutsättning för patientsäker vård. En i förväg genomtänkt plan och handlingsberedskap var av vikt när det kom till handläggandet av den oväntat svåra luftvägen. Genom ett ömsesidigt utbyte av information och kunskap kunde laget utveckla teamarbetet och förståelsen för varandras professioner.Background: When performing general anesthesia one of the most feared complications is hypoxia caused by blocked airways that may lead to devastating consequences for the patient. To improve patient safety it is important that teamwork is efficient and that communication is essential when working with patients. A small amount of research has been conducted regarding the anesthetic nurse’s experiences in dealing with an unexpected difficult airway, therefore it´s of importance to highlight this subject. In order to perform anesthesia there is a need of knowledge in how to take action when the difficult airway appear. Aim: The aim of this study was to examine the anesthetic nurse’s experiences and the importance of teamwork when dealing with an unexpected difficult airway. Method: A qualitative method was chosen to obtain a deeper understanding about the experiences regarding the subject. Data was collected through two focus group interviews with a total of eight participating anesthetic nurses. The interviews was transcribed and analyzed with the help of a qualitative content analysis. Result: A total of three categories with six subcategories each were developed, these described the anesthetic nurses experiences of dealing with an unexpected difficult airway. The identified categories were: communication, team capacity and preparation. Conclusion: Anesthetic nurses did value the communication and teamwork highly. The results showed that communication appeared different depending on how well the team members knew each other and that the effective communication was vital for the patient’s safety. A, in advance, developed plan of action was important regarding in how to handle an unexpected difficult airway. Through a mutual exchange of information and knowledge the team was able to develop the teamwork and understanding for each other’s professions.
